Please bring the following items to your visit:    Loyalty card (if previously issued)    Picture ID (driver’s license)    Information regarding others in your household (full names & birthdays)    Proof of household income such as a check stub, bank statement or Social Security statement    Proof of Effingham residency such as utility bill, with your name and physical address on it
